To initiate services or to schedule an appointment please click the link below. All appointments are subject to availability of an appropriate clinician. Appointments are only confirmed when clinician has accepted appointment, and you receive a confirmation via text or ​e-mail. 

Payment and Insurance Information

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental or behavioral health insurance benefits?
  • What is my deductible and has it been met?
  • Do I have a per visit co-payment?
  • How many sessions per year does my policy cover?
  • Is a referral required?​
  • Do I need a prior authorization?

Payment
You are responsible for all fees for services billed to your account. Insurance or EAP company failure to pay does not relinquish your responsibility to cover the cost of services. Billing will take place within 30 days of your monthly appointments or at the end of your EAP authorization period. Any remaining balance is the responsibility of the client. All co-payments are due at the time of session. A link will be available on your client portal to make your payment before your session starts using a valid debit or credit card. No checks allowed. There is a fee for all paperwork requested to be completed by a clinician.

Cancellation Policy
Must cancel appointment within 24 hours of your scheduled appointment time via Client portal. Failure to cancel within 24 hours or you do not show for scheduled appointment you will be required to pay a $100.00 cancellation fee per session missed. Payment will be made with card on file. There is a 10-minute grace period for all appointments. Please contact clinician for any emergencies.
